Answer: There is the 'S' shaped handle shaft piece. There is then a center shaft piece & the shoe horn piece. A total of 3 pieces. -

Answer: Just measuring with a vernier caliper it looks to be M14 - 2.00mm pitch, which is a standard Metric Course thread. I would suggest you obtain an M14 - 2.00 metric nut & see if it fits the thread on the shoe horn to verify. -
